How to choose between them

Start with your main symptom. Hot flashes, night sweats, poor sleep and mood changes call for a systemic cream, either one rubbed into the skin or a higher-dose vaginal cream like Alumei. If vaginal dryness, painful sex or urinary symptoms are the main problem, a low-dose vaginal estrogen cream may be all you need. Then check whether you need progesterone (you do if you have a uterus and use systemic estrogen), and compare the total monthly cost including any membership.
