“HRT cream” covers several different prescriptions. The two questions that sort them are where you apply it (skin or vagina) and where it acts (whole body or locally). Getting those right matters more than the brand.

Estradiol body cream (systemic)

Bioidentical estradiol in a cream base, rubbed into the skin of the inner arm, thigh or lower abdomen once a day. It is absorbed into the bloodstream and treats whole-body symptoms.

Progesterone cream

Prescription progesterone is usually given as an oral capsule because skin absorption of progesterone is unreliable for protecting the uterus. Over-the-counter "progesterone creams" are unregulated and not a substitute for prescribed HRT.

A note on safety

Transdermal estrogen, including creams, has a lower blood-clot risk than oral estrogen. For most healthy women under 60 or within ten years of menopause, current guidance from The Menopause Society is that the benefits of hormone therapy outweigh the risks. Estrogen products carry FDA boxed warnings, and women with a history of breast cancer, clots, stroke or liver disease need individual advice.
