Combined HRT cream review · Oestra® by Inner Balance
Updated October 2, 2026 · BestHRTCreams editorial team · We may earn a commission
Oestra is the cream at the centre of Inner Balance’s menopause program. It is one daily pump of bioidentical estradiol and micronized progesterone, applied vaginally. You can’t buy the cream on its own: it comes with a membership that includes physician visits, messaging and dose changes, at $199 a month for six months and then $99.50. That membership is what you’re really choosing, so this review starts with the program and then covers the cream.

How the Inner Balance program works
- 1
Health questionnaire
A short online quiz about symptoms, cycle and medical history. Independent reviews describe it as about five minutes, with labs optional rather than required.
- 2
Physician prescription
A licensed physician reviews your answers and sets your starting Oestra dose. There is usually no video visit before the prescription.
- 3
Pharmacy and shipping
A licensed U.S. compounding pharmacy prepares the cream. Each shipment covers about 90 days, while billing is monthly.
- 4
Adjustment period
The first three months are treated as an adjustment period, with a re-evaluation around month four.
- 5
Ongoing support
Visits, messaging and dose changes are included for as long as you stay a member.
The cream itself
| Component | Per pump | Role |
|---|---|---|
| Bioidentical estradiol | 3 mg (0.3%) | Replaces the estrogen lost in menopause |
| Micronized progesterone | 100 mg (10%) | Protects the uterine lining and supports sleep |
| Cream base | MCT oil and emulsifiers | Carries the hormones into vaginal tissue |

Like Alumei, Oestra is applied vaginally but dosed for systemic effect. It targets hot flashes, broken sleep, mood swings, brain fog and low energy as well as dryness. Vaginal tissue absorbs hormones efficiently, and MedlinePlus notes that even vaginal estrogen products carry the same class warnings as other estrogens. A combined cream like this needs a clinician who knows your history.
How it compares with Alumei
Oestra’s per-pump strength works out to the same estradiol-to-progesterone ratio Alumei publishes per gram, and both use a $199 → $99.50 price step. The practical differences are price and checkout. Alumei advertises $179 and $0 due until a physician approves. Inner Balance puts more emphasis on its specialist support team. Our Alumei HRT cream reviews page has the full side-by-side.
The real cost of Oestra
| Period | Monthly | Total |
|---|---|---|
| Months 1–6 | $199 | $1,194 |
| Months 7–12 | $99.50 | $597 |
| Year one | - | ≈ $1,791 |
| Each year after | $99.50 | ≈ $1,194 |
The “from $99.50/month” headline you may see in ads is the price after month six. The cream, physician visits, messaging and dose changes are included, and there is no insurance billing. Reports on HSA/FSA acceptance differ between sources, so check at checkout if you plan to use those funds. For a lower-cost single-hormone option, see our Effecty estradiol cream review, where the cream costs from $65 a month with progesterone included.
The fine print: guarantee, cancellation and support
- Six-month guarantee. As summarised by independent reviewers, a refund claim must be filed within six months of the order and within 14 days of cancelling, and it is limited to one per customer.
- Cancellation. Some customers report a quick refund. Others, in BBB and Trustpilot complaints, describe subscriptions that were hard to cancel. Keep written records.
- Support channel. Support is advertised as unlimited, but some complaints say it was chat-only. If you want a video visit, ask before you enrol.
- States. Inner Balance’s own pages list different coverage, from 43 states plus DC to all 50. Confirm yours at signup.
Pros and cons
✓ What we like
- ✓Estradiol and progesterone combined in one daily pump
- ✓Strength published per pump, so you know your dose
- ✓Visits, messaging and dose changes included in the membership
- ✓Price halves to $99.50/month after six months
⚠️ What to watch
- •Highest starting price of the creams we rank ($199/month)
- •Six-month guarantee comes with a strict 14-day filing rule
- •Complaints about cancellation and chat-only support
- •Compounded, not an FDA-approved product
Is Oestra right for you?
Oestra makes sense if you have a uterus, have whole-body symptoms plus dryness, and expect to stay on HRT for more than a year, which is when the $99.50 rate pays off. It makes less sense if you want to try HRT for a month or two, want an FDA-approved product, or want a video consultation before you are prescribed anything. See all options in our HRT cream comparison.
Editorial verdict
Oestra is a well-formulated combined cream, and the long-term price of $99.50 a month is competitive. The score of 9.21 reflects both sides. The cream and the support model are strong, but the six-month $199 commitment and the strict refund terms mean you should read the cancellation policy before entering a card number.
